- Implantable Loop Recorders (ILR) are miniaturized devices that allow continuous ECG monitoring over several years and can therefore provide an optimal correlation between symptoms and the documented cardiac arrhythmia. Accordingly, ILR implantation is used to clarify unexplained syncope, but also to detect asymptomatic atrial fibrillation, e.g. after ablation or after cryptogenic stroke.ILR detection is programmed according to the size of the ventricular signal in the ECG and the indication or optimal detection of the suspected arrhythmia. After initial programming, the follow-up is frequently carried out by telemedicine.In this Austrian consensus recommendations for both programming and follow-up are given based on expert opinion. Particularly, indications for ILR implantation after syncope and ischemic stroke are proposed using a novel algorithm.
