- In people with type 1 diabetes, regular physical activity is associated with positive effects on the cardiovascular system and all-cause mortality and is, therefore, an essential component of modern diabetes management. However, the underlying insulin deficiency and the need to administer exogenous insulin contribute to a dysbalance of physiological glucose homeostasis which is associated with acute problems such as hypoglycemia, especially in the context of exercise. Specific adaptations of insulin therapy, both during exercise and in the warmup and recovery phase, depend on the type, duration, and intensity of physical activity and require individualized therapy recommendations that are supported by appropriate scientific evidence. The influence of physical activity on (patho)physiological mechanisms of glycemic control in people with type 1 diabetes and the current status on recommendations for insulin therapy adjustments in response to physical exercise are presented.
