Abstract:
This diploma- thesis is a retrospective data- analysis of 21 patients with surgical treatment of the upper cervical spine. Trauma, Tumor and Rheumatoid Arthritis can lead to instability of the cervico- cranial- function. Restoring stability and securing of neurological structures is the goal of surgery in these cases. Given that indications for surgery are based on dislocation and neurological symptoms, we want to point out specific features of the surgical technique and demonstrate its influence on the post- surgical outcome. Our data show good to excellent outcome in most cases without influence of postoperative complications or pre- existing neurological impairment. Relief of pain is the main subjective benefit. Reduction of mobility limits subjective outcome score but does not overlay total outcome results. Specific information either for possibly affected patients (e.g. in Rheumatoid Arthritis) as well as for physicians on affection of the cervico- cephalic region is crucial. In cases of chronic instability as seen in Rheumatoid Arthritis and cases of non- union after fractures of the Dens, prospectively proven data on outcome after surgery are missing. Thus, further data has to be acquired and analyzed to justify surgical treatment.
