- Patients with chronic heart failure are rarely in a stable condition. Depending on their functional impairment they often suffer from acute cardiac decompensation. As this is the most costly part of the disease several efforts are made to give the patient stability: Optimizing therapy during the hospital stay is mandatory. However, special disease management programmes are more and more implemented to guide patients after discharge. Amongst them telemedicine is most promising. In this case telemedicine describes analysis of data (measured by the patient and transmitted to the treating physician) with special respect to early warning signs of new episodes of cardiac decompensation. One such project is MOBITEL (MOBIle TELemonitoring for heart failure patients) which has been done under the auspices of the Department of Cardiology at the Medical University Graz.
MOBITEL investigates the possible influence of a mobile-phone-based telemonitoring system for patients during the first six months after cardiac decompensation on outcome. On a daily base patients measure vital parameter such as blood-pressure, heart rate and body weight, they transmit these data together with doses of daily heart failure medication and an estimation of their wellbeing. The main purpose of MOBITEL is the investigation whether or not a mobile-phone-based telemedical surveillance of simple vital parameters improve prognosis compared to a controlgroup without this service.
As MOBITEL investigates therapy-monitoring I will first describe heart failure itself, its pathophysiology , therapeutic approaches and, finally, telemedicine and the study MOBITEL in detail.
