Abstract:
Emergency ultrasound is a well-established tool for immediate bed-side diagnostics in the acute and critically ill. In the prehospital setting, small battery-powered devices can be used by emergency physicians directly at the scene or in the ambulance. Quick and focused ultrasound examinations have been developed to provide answers to specific questions. Lung ultrasound can help to identify common causes of acute dyspnea within minutes. During CPR, sonography can detect reversible causes and be an important marker for cessation or continuation. Pericardial tamponade, pneumothorax, intraabdominal or intrathoracic bleeding and elevated intracranial pressure can be diagnosed in trauma patients allowing immediate treatment or rapid transportation to the appropriate hospital. Emergency echocardiography helps to diagnose the underlying causes of hypotension, shock or thoracic pain and can be used to assess preload and volume status. Furthermore, ultrasound guidance can be extremely helpful in establishing peripheral and central vascular access. There is strong empirical evidence for the use of intrahospital emergency ultrasound. The use of ultrasound has also grown in prehospital emergency medicine in recent years and first promising case reports and studies have been published.
