- Background: Osteomyelitis and aseptic necrosis (AVN) are diseases, which can lead to developmental disorders of the musculosceletal system. In special cases as e.g. patients with therapy-resistant chronic osteomyelitis, but also for AVN patients the therapy possibilities are not sufficient enough to protect children or adolescents from disturbance of growth. Material and methodes: To gain further information regarding the benefits of medicating children and adolescents suffering of osteomyelitis and aseptic bone necrosis with the hyperbaric oxygen therapy (HBO) a retrospective survey was carried out at the department of thorax surgery in co-operation with the department of child-orthopedy and the clinic for child surgery in Graz. Patients between 5 and 22 years old suffering of osteomyelitis or aseptic necrosis treated between 2002 and 2008 were identified on basis of the patient documentation system MEDOCS and called up for further examinations. Within the framework of this study the prior mentioned patients were compared to a matched control group. Results: In general no significant advantages from additive HBO therapy could be recognised. However it should be considered that patients of the HBO-group were showing more complex and severe disease patterns. Furthermore the comparison of the patients was difficult due to the high heterogeneity of the group. Negative effects of the HBO therapy and/or side effects could not be identified. Conclusion: In the literature there are already positive results regarding medicating above mentioned disease patterns with HBO. However these results refer to adult patients. So far the HBO is only regarded as an additive method in the treatment of the osteomyelitis and AVN and the indication is currently set only on basis of intention to treat . In order to obtain a more accurate statement regarding the effectiveness of HBO in childhood and adolescence a prospective and randomized survey should be carried out with a larger group of patients.
