Quality of life of patients with a chronic sinus tract in the hip and knee joint due to periprosthetic joint infection

- Introduction
The aim of this study was to evaluate the quality of life (QOL) and the functional outcome in patients living with a natural or surgical sinus tract due to treatment-resistant periprosthetic joint infection (PJI).
Methods
A follow-up examination was performed at the Department for Orthopedics and Trauma of the Medical Hospital Graz. We retrospectively included patients with a chronic sinus tract due to treatment-resistant PJI. The QOL was measured with the Hospital Anxiety and Depression Scale (HADS-D/A), the Visual Analogue Scale (VAS) and the Short Form-36 (SF-36) score.
Results
In total, 16 patients were included, with a mean follow-up time of 51.8 (± 23.9) months. The mean SF-36 Physical Component Summary (PCS) was 26.3 (± 8.2) and the Mental Component Summary (MCS) was 55.0 (± 9.4). The mean HADS-D was 8.0 (± 3.5), the HADS-A was 6.0 (± 3.8) and the VAS was 5.1 (± 2.6). In total, the chronic sinus tract had to be reopened in nine out of 16 patients (56.3%), while five patients (31.3%) were able to live with a chronic sinus tract.
Conclusion
The present study indicates that a chronic sinus tract is a treatment option for selected patients with treatment-resistant cases of PJI. The treatment provides an acceptable QOL despite functional limitations. It can be considered for patients with multiple diseases and high surgical risk or if the bone and soft tissue conditions prevent surgical intervention.
