- Introduction
The most established anesthetic procedure during total trapeziometacarpal joint arthroplasty is the regional anesthesia. By comparison, local anesthesia theoretically provides advantages in overall cost, in having the option for an intraoperative assessment of the prosthesis and in minimizing the anesthetic risk. In this study we compare local to regional anesthesia and analyze the outcome, safety, and overall patient satisfaction.
Methods
In this single-center, retrospective cohort study 32 patients (34 operated thumbs), that underwent total TMC-joint arthroplasty between February 2018 and July 2021, were included. Two groups were formed depending on the anesthetic method used. One group was operated under local anesthesia (LA), and the other one under regional anesthesia (RA). Information regarding function, pain and hospital discomfort was gathered with data from a prospective study. Information regarding intraoperative pain and anxiety, overall satisfaction, pain medication and willingness to repeat the procedure was gathered via a retrospective telephonic survey. A statistical analysis of differences between the two groups was conducted with SPSS.
Results
Our analysis shows significant differences regarding higher intraoperative anxiety in the LA-Group, and a longer set-up time and longer operation length in the RA-group. Both procedures were reliable with no significant differences regarding intraoperative pain. No conversion from either anesthetic method to another was necessary.
Conclusion
The use of local anesthesia is a safe and reliable alternative to the well-established regional anesthesia. Local anesthesia reduces the cost of the procedure and the time of the patient’s hospital stay. The patient should be involved in the selection of the anesthetic method to optimize the operative procedure and the outcome.
