Prospective randomized study comparing the outcomes of prosthesis of the thumb saddle joint to resection-suspension arthroplasty

Abstract:
Introduction: Rhizarthrosis is a common disorder of the thumb with a frequency of 10%, especially in the older female population. There are many conservative therapies like splints, nonsteroidal anti-inflammatory drugs (NSAIDs) corticosteroid – or hayluronacid injections, but if they are exhausted, surgical treatment is recommended. The purpose of this study was to compare the outcome of the Moovis prosthesis of the company Stryker and the resection suspension plastic according to Epping regarding pain, mobility and quality of life. Method: In this randomized, controlled study, two patient groups aged from 42 to 83 (median 57) with osteoarthritis of the trapeziometacarpal joint were compared. 100 operations, from a patient collective of 81 women and 12 men were equally divided into two groups, group one the Moovis prosthesis and group two the resection arthroplasty (RA) with tendon interposition in the technique of Epping. The data regarding agility, pain, patient satisfaction and quality of life were collected and evaluated with the help of questionnaires as well as standardized examination prior and three, six and twelve months after the operation. Results: With all patients we could see an improvement in all scores in the observation period, no matter what kind of surgery was chosen, except a worsening in the 12-month HADS-Score in the Epping-group. There were significant improvements with the prosthesis in the group comparison in terms of VAS (p=0.034), DASH-Score (p=0.036), HADS-Score (p=0.026), PCS (p=0.014) and retropulsion (p=0.036). Conclusion: In our study with a collective of 98 controlled hands, we saw superior results regarding pain and function in the group treated with Moovis prostheses after 12 months. However, no reoperation was required in the Epping group, but three in the prosthesis group. Long-term studies are necessary for further evaluation of longterm outcome.
