Abstract:
Introduction: Pelvic organ prolapse is a common problem and has a high impact on the quality of life and the sexuality of affected women. Up to 25% of all women suffer from pelvic organ prolapse. Reasons are multifactorial, but the most frequent risk factors are childbirth, age and obesity. If conservative therapy fails, women have to undergo surgery. The aim of the master thesis is to determine the change in sexuality and quality of life three months after surgery after a sacrospinous ligament fixation (Amreich- Richter) vs Mc Call Culdoplasty in the course of vaginal hysterectomy for prolapse. Methods: This master thesis is designed as an intermediate analysis of an ongoing randomized controlled study. Women who underwent vaginal hysterectomy for prolapse were randomized to Amreich- Richter or Mc Call Culdoplasty. Data concerning sexual health and quality of life were compared three months postoperatively. Preoperative patients underwent a medical history and clinical examination and completed the PISQ-IR (Pelvic Organ Prolapse/Urinary Incontinence Sexual Questionnaire- IUGA Revised) for sexuality and the King’s Health Questionnaire (KHQ) for quality of life collection both pre- and postoperatively. Results: 71 women are included in this master thesis, 37 (52%) were randomized to the Amreich- Richter technique, 34 (48%) to the Mc Call technique. According to both questionnaires a difference in quality of life and sexuality could be observed in both study-groups. The number of sexually active women increased in both groups 3 months after surgery. Less women suffered from dyspareunia in the Amreich- Richter- group three months after surgery. Conclusion: Sexual health and quality of life improved in both groups three months after surgery. A final statement can be done when all patients are evaluated.
