Abstract:
Abstract Backround The determination of corneal refractive power after refractive surgery is considered one of the most important parameters for the correct determination of the intraocular-lens (IOL) power. The implantation of an IOL, for example, is necessary for clinically apparent cataracts and this procedure will become more common over time due to demographic change. In order to find a way to correctly determine the refractive power after SMILE surgery, even without preoperative data, various values were determined using Pentacam (Oculus, Lynnwood, Washington, USA) and compared against the “gold standard”, the Clinical History Method (CHM). Methods In this retrospective study, 88 eyes of 46 patients were operated for myopia correction. 45 eyes were operated using Relex-SMILE and 43 eyes using LASIK. Postoperatively we measured the K-values, true net power (TNP), total corneal refractive power (CRP), and equivalent K-values (EKR) at 1; 2; 3; 4; 4.5 and 6 mm through the use of an Oculus Pentacam. These values were tested for statistically significant differences between the two groups, as well as for CHM differences in the respective group. Results The CRP and EKR 4 values within the SMILE group had the smallest difference when compared to CHM but nonetheless showed a statistically significant difference. After LASIK, TNP and CRP had the least difference in comparison to CHM. Notably TNP was the only value which showed no statistical difference when compared to CHM. Conclusion Since the CRP values of the SMILE group had the smallest difference to the CHM, it is recommended to use the CRP values for biometry according to SMILE, if no preoperative data are available. After LASIK, the use of TNP for biometry is recommended.
