Abstract:
Morbidity and mortality due to malignant tumors of the liver and extrahepatic bile ducts are in the “top ten” around the world and it confirms a need to improve prevention measures, highlights unsatisfactory results of treatment of malignant tumors of these locations and requires further improvement of treatment. The aim of my scientific work is to analyse the immediate and long-term results of treatment of patients and, taking into account the data obtained from the analysis, to make recommendations for a selection of an optimal strategy to treat these groups of patients. Retrospective analysis of 658 operated patients with malignant tumors of the liver and extrahepatic bile ducts had been conducted at the Department of Liver and Pancreatic Tumors at N.N. Blokhin Russian Cancer Research Center from 1990 to 2014. The analysis covered the outcome for those patients with synchronous / metachronous metastases in the liver; solitary, isolated or multiple metastases; the size of the metastases; lobes affected; and differing chemotherapy. Statistical analysis of the data was performed by using GraphPad Prism 5. Survival curves were constructed according to Kaplan-Meier's method. Differences between parameters were statistically significant in case of p <0.05. The outcomes of the treatment are unsatisfactory and detailed separately in each of these three scientific works and are comparable with the unsatisfactory results of treatment of these types of patients according to the works of other authors. At the end of each scientific work there are proposals to optimize the treatment in order to improve the outcome of treatment. These proposals were applied in treatment practice of the Department of Liver and Pancreatic Tumors at N.N. Blokhin Russian Cancer Research Center. In the future it is planned to carry out a further analysis of these proposals implemented in the practice of malignant tumors of the liver and extrahepatic bile treatment again.
