The Austrian Transcatheter Aortic Valve Implantation (TAVI) Registry - 3 Years' Data

Abstract:
Background/Objectives: Transcatheter aortic valve implantation (TAVI) is a new intervention for the treatment of patients with severe aortic stenosis. The Austrian TAVI registry documents the baseline characteristics, procedural risks and outcome of TAVI implantations in Austria. Methods: The registry was launched on 1 January 2011, to monitor the outcomes of the TAVI procedure performance in Austria and to provide data for clinical studies. Results: Between January 2011 and December 2013, data of 959 patients (mean age 82±5 years) entered the registry. Logistic EuroSCORE was 21.4±13.2% and STS-Score 15.4±11.0%. After baseline assessment and documentation of the procedure, the registry guidelines recommended follow-up visits at 1, 3, 6, 12, 24 and 36 months. The preferred access site was transfemoral (n=862). The acute procedural success rate was 94%. Complications arose in 24% of cases. Follow-up showed significant improvement of hemodynamic parameters (vmax 4.5±0.7m/s vs 1.9±0.5m/s, mean gradient 51±16mmHg vs 9±4mmHg, p<0.001). Frailty, assessed with the Karnofsky index, showed significant improvement from 60±10% to 80±20%, p<0.001. Periprocedural mortality was 1.7% (16 patients). Thirty-day survival was 94.9%; one year censored survival was 87.4%. Conclusion: Data from this registry prove successful implementation of results from studies for TAVI in routine clinical settings. TAVI showed a high success rate with acceptable complication rates. Optimal patient selection remains crucial.
