Abstract:
Heart failure (HF) is a major public health and economic problem worldwide. The absolute number of patients with HF will continue to increase. The echocardiographic study is essential to characterize the underlying functional and structural changes especially as related to diastolic function and clinical outcomes in HFPEF. Tissue-Doppler derived E/e′ ratio constitutes as one of the cornerstones in the non-invasive evaluation of diastolic function at rest. Of all echocardiographic parameters investigated, the LV filling index E/e′ was identified as the most sensitive indicator to detect impaired filling and elevated filling pressures in suspected HFPEF patients. At the moment the diagnostic conception of HFPEF relies on resting examinations. However, many patients with signs and symptoms of heart failure with preserved ejection fraction fall into the grey zone of key echocardiographic diagnostic parameters, such as E/e′ = 8-15. But in patients with early-stage HFPEF (in whom such symptoms as dyspnea and fatigue appear and filling pressure increases only on exertion) E/e′ ratio at rest may be insufficiently sensitive to identify them. Some studies demonstrated that additional alterations in HFPEF patients occur only during exercise. Therefore the primary objectives of the study are to assess whether a) novel echocardiographic imaging techniques allow a better diagnosis of HFPEF; b) the diastolic stress test allows the diagnosis of diastolic heart failure in HFPEF patients in the diagnostic grey zone. The Diagnose-DHF Pilot trial is the first pilot trial testing the value of new imaging modalities (myocardial deformation at rest and during diastolic stress-test) in patients with HFPEF, hypertensive heart disease (HHD) and healthy volunteers.
