The spectrum of dermatological diseases during pregnancy - An overview of 10 years at the specialized dermatological pregnancy outpatient clinic

Abstract:
Objective The purpose of this study was to get an overview of dermatological diseases in pregnant females, who consulted the specialized dermatological pregnancy outpatient clinic at the university hospital in Graz, Austria, over a period of 10 years. Methods A retrospective data analysis of medical records including history, clinical findings, diagnostic procedures, therapeutic approach and referral status was performed of 486 pregnant patients with skin diseases, seen between 2003 and 2013. Results The majority of patients (n=257; 53%) presented with a so-called ‘specific dermatosis of pregnancy’. 41,8% (n=203) were diagnosed with atopic eruption of pregnancy (AEP), 7% (n=34) with polymorphic eruption of pregnancy (PEP), 4% (n=19) with intrahepatic cholestasis of pregnancy (ICP) and only 0,2% (n=1) with pemphigoid gestationis (PG). AEP developed significantly earlier in pregnancy (59% < 3rd trimester) compared to ICP and PEP, which mainly occurred in late pregnancy. Striae distensae (91%) and trunk involvement of skin lesions (97%) characterized PEP, whereas occurrence of exclusively secondary skin changes (88%) and, in particular, generalized pruritus (89%) were typical for ICP. The remaining 47% of pregnant females presented with non-specific dermatoses unrelated to pregnancy. This group of dermatoses showed a significant higher incidence in early pregnancy (65% < 3rd trimester) in contrast to the specific dermatoses of pregnancy. More importantly, pruritus was reported in remarkable 100% of patients with specific dermatoses compared to 49% with miscellaneous skin diseases coinciding with pregnancy. Conclusion If expecting women present with pruritus, primarily the specific dermatoses of pregnancy need to be ruled out. AEP is the most frequent specific dermatosis and mainly develops in early pregnancy. In contrast, PEP and ICP occur rather late in pregnancy.
