Possibilities and results of the Delta inverted shoulder prosthesis

- Aim: The rational of using as inverted shoulder prosthesis was the aim to improve the functional outcome and patients satisfaction in painful CTA shoulders post-operatively. In a retrospective controlled study we were evaluating our results after implantation in patients with pre-existing rotator cuff tear in addition to subcapital fractures, recurrent dislocation or omarthritis with concomitant degenerative changes, a condition that mostly affects the elderly. Material and Methods: Form 12/02 until 03/05, 22 Delta prosthesis were implanted in 20 patient. Out of them 17 implants in 16 patients, including 12 females (average age 74,9 ys) and 4 males (average age 70,1 ys ) were part of our study. After a mid range follow-up period of 13,7 months (range, 4 to 27 months) patients were controlled radiologically and clinically using the age and gender related Constant Score and the DASH Score. All obtained data were compared with pre- and post-operative data and x-rays, inferred from operation and ambulance records. Results: The overall clinical results in all patients (degenerative and posttraumatic with degenerative changes) showed a mean Constant Score of 48.9 points. The mean points for pain were 11.3, for activity 12, mobility 19.3 and strength 6.4. Overall our patients rated 67.5 n the DASH Score. These findings are in line with good short-term results previously reported in the literature with this prosthesis. Concerning their age, patients got 67.9% shoulder function of normal, healthy shoulders. By drawing a comparison with pre- and post-operative values, we got highest significant improvements of all of our results. Unaffected or at least reasonable sleep of 81.3% and absolute satisfaction of 94.1% of our patients could prove as well a substantially increased life quality as an absolute alternative in the treatment of rotator cuff defects in connection with defectarthropathies. From the radiological point of view at the time of follow-up 70.6% showed glenoid notching, but more suggestive of osteophytic bone transformation than real bone erosion but of one failure with signs of real loosening that required later revision. In the case of complication, the Constant Score dropped to 10 and the DASH Score rated 99. Except this, the image remained stable makes a transversal sliding in the first view degrees of abduction that even later turns to a rotation. This is especially the fact for artificial glenoids larger than 36mm.
