Abstract:
A new additional therapy of keratoconus is photooxidative crosslinking of corneal collagen with riboflavin and UV-A-light. The method is currently undergoing clinical evaluation worldwide especially concerning late complications. In this study we confirm collagen crosslinking by formation of stable gels from corneal collagen solutions during irradiation with riboflavin addition. However the hyaluronate molecule, a representative glycosaminoglycan of the cornea, is degraded to small fragments by UV-A irradiation with addition of riboflavin as shown by viscometry. Analysis by polyacrylamidegel electrophoresis shows that also tear fluid proteins are destroyed by this method. This may explain the occasionally occurring haze of cornea and glare sensitivity of patients after photo-oxidative crosslinking therapy of keratoconus.
