Abstract:
Prospective randomised clinical trials have the highest level of evidence and therefore help in effective, efficiant and safe therapy. The contents of these landmark studies (ocular hypertension, early glaucoma, advanced glaucoma, normotension glaucoma, initially diagnosed glaucoma with medical vs. operative therapy) are demonstrated and their practical relevances summarized. Most of the earlier studies had a decrease of the intraocular pressure as main study outcome and not changes of the visual field or the optic disc. All landmark studies showed, that a lower intraocular pressure helps to delay or stop functional and/or structural changes. All known risk factors, which could be found in unique constellations in each patient, should be taken into account. For the first time the natural course (without any therapy or with placebo) of the disease was investigated.
