Abstract:
About 6 to 8% of the normal population is sensitized to nickel, and around 25% of the eczema patients react positive to nickel in the patch test. Due to the ubiquitous occurence in the environment and the permanent contact with nickel-containing products in many occupations, nickel allergy is usually regarded as being caused by occupational contact and being relevant for any case of skin problem at the workplace. If the sensitization is detected before entering a certain occupation, vocational guidance usually advises an applicant against many professions, and the clinical relevance is often regarded as being undebatable. Epidemiological surveys, material studies and inspection of workplaces however somehow contradict those old general opinions. Nickel sensitization is only infrequently acquired through a certain job, and the significance of the finding often estimated too high. Primary prophylaxis (regulation of imitation jewellery), information and protective measures at the workplace could all help to avoid many problems, and the judgement in questions of vocational disease should usually be more critical. Nickel sensitivity should not be regarded as handicap for many occupations, and the knowledge within the medical faculty deserves some reevaluation.
