- We report on a 23 months old girl with a 7 cm long floating thrombus in the inferior caval vein with dramatically increasing risk of lethal pulmonary embolism under anticoagulation with heparin. Surgical removement of the thrombus or implantation of a vena cava filter was not possible. Fibrinolytic therapy with recombinant tissue plasminogen activator dissolved the thrombus without any signs of pulmonary embolism. This case report can provide useful information for other children with similar problems.
