- Background. Adequate medical treatment of acute viral bronchiolitis in infancy is often a problem for both practitioners and hospital-based pediatricians. Causal treatment options are lacking;thus only symptomatic therapy is available. Therapy. The effectiveness of inhaled bronchodilators is substance to controversial discussions. Some centers use them routinely, but most studies have failed to document a generalized therapeutic efficacy. Steroids for mild and moderate severe bronchiolitis seem to provide no clinical improvement; however, some infants with severe disease may benefit from their use. Antiviral substances like ribavirin are ineffective for the treatment of RSV-induced bronchiolitis. Antibiotics are rarely indicated;their use should be limited to exceptional cases of bacterial super- or coinfection. Supplemental oxygen and adequate hydration represent the mainstays of adjuvant therapy. Other options like application of secretolytic agents, mist therapy and chest physiotherapy remain popular; a clinical benefit, however, has not been demonstrated. Some of these strategies may even worsen bronchoconstriction.
