Pneumothorax and pneumomediastinum in an 8-year-old boy

Abstract:
We report an 8-year-old boy admitted to hospital because of recurrent cough attacks and thoracic pain. The child presented with signs of a respiratory infection. Chest X-ray revealed left-sided pneumothorax without the indication for invasive therapeutic intervention. The patient was monitored by repeated chest X-rays. With ongoing attacks of coughing, pneumomediastinum and cutaneous emphysema of the neck developed. After progressive improvement in clinical symptoms and radiological changes, he presented with total atelectasis of the left lung. Flexible bronchoscopy revealed aspirated chewing gum occluding the left main stem bronchus. After its removal,the pneumothorax and pneumomediastinum rapidly disappeared. Spontaneous pneumothorax is rare in childhood; foreign body aspiration should be included in the differential diagnosis.
