Tarsal metastasis as the primary manifestation of endometrial cancer. A case report

- Introduction: Isolated distant metastasis at the time of diagnosis of endometrial cancer is rare. We report on a patient with long-term survival after presenting with a solitary tarsal metastasis of an endometrial cancer.Case Report: A 61-year-old patient presented with pain and swelling of the left foot, Radiography and bone scintigraphy showed a cystic lesion of the left calcaneus and talus. Open biopsy showed endometrial adenocarcinoma. Transvaginal ultrasonography was suspicious and curettage confirmed endometrial carcinoma. All other studies were normal. Abdominal hysterectomy with bilateral salpingo-oophorectomy, pelvic and para-aortic lymphadenectomy, and amputation of the lower left leg were performed. Histology of the uterus and the metastasis in the tarsus showed a well differentiated endometrioid adenocarcinoma. Postoperatively, the patient received six courses of platinum-based chemotherapy and medroxyprogesterone acetate orally for 5 years. The patient is alive and free of disease 96 months after the primary diagnosis of the tarsal metastasis.Conclusion: Radical surgery and adjuvant chemotherapy with a curative intent appear warranted in patients with a solitary bone metastasis of endometrial adenocarcinoma.
