- Malignant changes are seen in 0.2% to 1.8% of mature teratomas of the ovary and only 15 cases of primary malignant melanoma of the ovary, with survival times between 4 and 60 months, have been reported in the world literature. We describe a 70-year-old patient who was found to have a primary malignant melanoma associated with a mature teratoma and two liver metastases. Total abdominal hysterectomy with bilateral salpingo-oophorectomy and resection of the hepatic segments III and IV were performed. The patient received adjuvant chemotherapy with vincristine, dacarbazine, cyclophosphamide and doxorubicin but died of disease at 16 months. The literature suggests that surgery followed by platinum-based chemotherapy, dacarbazine and immunotherapy may improve the unfavorable prognosis of patients with this condition.
