Abstract:
The development of specific clotting factor concentrates has made possible surgery ill patients with congenital disorders of haemostasis, but a number of prerequisites and precautions have to be met: Only with an exact diagnosis specific replacement therapy can be administered. All patients must have inhibitor testing prior to surgery, because a high-titre inhibitor makes replacement by means of specific clotting factor concentrates impossible. Hepatic disease increases the risk of thromboembolic complications during replacement therapy and, therefore, should be looked for. Enough factor concentrates must be ready, and the laboratory should be prepared to test the effects of replacement therapy. Finally, specific surgical procedures can reduce the risk of bleeding in patients with a congenital bleeding disorder. Replacement therapy is done with specific high-purity factor concentrates by intermittent bolus injection or by continuous infusion, inmost cases until completion of wound healing.
