Abstract:
Background: Adults with congenital heart disease (ACHD) are a growing and highly complex patient population. While over 95% of children with congenital heart disease now reach adulthood, many are lost to specialized follow-up after pediatric care. The ACHD frequently suffer from complex comorbidities. Their care requires expertise beyond cardiology. Objective: Presentation of the importance of interdisciplinary, lifelong care of ACHD and the key role of pediatricians in early detection, structured transition and coordination of long-term care. Methods: Narrative review based on the 2020 ESC guidelines for ACHD, data from the German National Register for CHD and recent literature on comorbidities, quality of life and care outcomes. Results: In Germany 80% of ACHD are not seen in specialized centers. Multiple system comorbidities, such as of the kidneys, liver, nervous system and endocrine system, occur earlier than in the general population and often go unrecognized. An interdisciplinary care involving nephrology, hepatology, anesthesiology, psychology and social services is essential in order to cover the spectrum of care required. Pediatricians play a crucial role in initiating structured transition processes and the interprofessional collaboration. Conclusion: The management of ACHD must be interdisciplinary and personalized based on the individual heart defect and the comorbidities. Continuous and close collaboration between pediatricians, general practitioners, cardiologists and other specialist disciplines is vital for lifelong, holistic care.
