- The transconjunctival approach is an established procedure in oral and maxillofacial surgery to perform surgical interventions in the region of the orbit. The wide range of indications includes not only traumatology as well as treatment of tumor diseases of the facial skull and congenital facial malformations, but increasingly also treatments to restore and optimize facial aesthetics. The approach combines good intraoperative visibility of the surgical field with an inconspicuous postoperative periorbital appearance without externally visible scars. In addition to the above-mentioned advantages of the transconjunctival procedure, however, this surgical approach is also associated with relevant complications, the control of which must be guaranteed at all costs. The complication profile includes the occurrence of eyelid malposition (entropion or ectropion), canthal malposition, chemosis or symblepharon. The aim of this work is to highlight the frequent complications in the context of the transconjunctival approach and to elaborate their management in detail, in order to create a practice-relevant overview that has not yet been described in this form in the literature.
