Abstract:
Up to 85% of all peri- and postmenopausal women are affected by climacteric symptoms such as hot flashes, sleep disturbances, depressive mood disorders, cardiac symptoms, and urogenital problems. Nevertheless, only 3% of those women are treated with a menopausal hormone replacement therapy (MHT). This is mainly related to the publication of the Women's Health Initiative (WHI) study in 2002. WHI results suggested that there is an association of MHT with an increased risk for cardiovascular disease as well as for invasive breast cancer. It should be pointed out that the women had a mean age of 63 years when treatment started. Moreover, the study medication included a synthetic gestagen. In women < 60 years, the authors found a decreased risk of colon cancer, fractures, type 2 diabetes mellitus, as well as for mortality. Starting MHT in women < 60 years and/or within 10 years of the final menstrual period (FMP) seems to be safe and effective. MHT is indicated when women suffer from climacteric symptoms. MHT includes a combined treatment with 17 beta-estradiol (orally or transdermal) and natural progesterone (the use of synthetic gestagens should be avoided because of an adverse risk-benefit ratio). An estrogen-only therapy is only indicated in women after hysterectomy. Women with early (FMP < 45 years) or premature menopause (FMP < 40 years) should be treated with a hormonal replacement therapy (HRT). HRT is useful regarding the prevention of chronic diseases such as cardiovascular diseases as well as osteoporosis.
