- Fever and signs of an inflammatory disease are among the most common symptom complexes that lead to an acute pediatric presentation. The generally accepted ideas about the causes and manifestations of fever, about physiological and pathological immunological processes in inflammatory reactions and the therapeutic measures derived from them are subject to constant change. Patients as well as those providing treatment are influenced by conceptual considerations which, from a historical point of view, are continuously evolving. In this review article the knowledge on immunological processes underlying fever reactions and inflammation are illustrated. On the basis of historically relevant medical milestones with mainly phenomenological descriptions as well as some outstanding steps towards a modern understanding of immunological mechanisms, the ideas on defence against infection and also the modern concepts of autoimmunity and autoinflammation are summarized. In the present time, significant efforts are being undertaken to translate the knowledge of molecular mechanisms into clinically applicable therapeutic principles. The increasingly complex explanatory models will contribute to the further development of inflammation medicine in the future with potentially targeted therapies, personalized precision medicine and technological innovations.
