- Background: Currently young surgeons are puzzled with the decision of whether to become a surgeon-scientist and to actively pursue clinical and basic research in an academic institution. Methods: This review is based on a personal experience and a literature PubMed search. Results: The experiences of a surgeon with research in Europe and the USA discuss the differences in systems, as well as the opportunities, challenges and hurdles of surgical research. The desirability of the USA amongst European scientists was found to be largely based on superior resources and funding opportunities. Conclusions: Despite all difficulties that surgical research may be facing to date becoming a surgeon-scientist and performing basic research in Europe and the USA can be a mutually beneficial situation, significantly broaden one's perspective and offer unique opportunities in a surgeon's career.
