- Liver disease may be complicated by pulmonary vascular diseases. Portopulmonary hypertension (POPH) is caused by a proliferative vasculopathy affecting small pulmonary arteries, causing an increase in pulmonary vascular resistance and right heart strain. In contrast, hepatopulmonary syndrome (HPS) is caused by pulmonary vascular dilatation and arterio-venous malformations leading to a ventilation/perfusion mismatch. Both conditions are associated with increased mortality and poor quality of life. POPH is diagnosed using echocardiography and right heart catheterization and can be treated with pulmonary vasoactive drugs. HPS is diagnosed by means of contrast enhanced echocardiography and blood gas analysis. Currently, there is no approved drugs for HPS. In case of liver transplantation, care must be taken to optimize management of patients with coexisting pulmonary vascular disease.
