Abstract:
Severe pulmonary hypertension has a poor prognosis and is complicated to treat. In primary pulmonary hypertension and pulmonary thromboembolism the pulmonary vascular resistance will determine the prognosis. In left ventricular failure and in chronic cor pulmonale the pulmonary resistance is a prognostic factor and its increase may be the main cause of clinical deterioration. In mitral valve surgery and heart transplantation the results depend in great parts on preoperative pulmonary vascular resistance. In ARDS right ventricular failure due to pulmonary hypertension is a common cause of death. In this paper the pathophysiologic mechanisms leading to pulmonary hypertension and right ventricular failure as well as current therapeutic concepts and results are discussed.
