- The new European Society of Cardiology/European Respiratory Society (ESC/ERS) guidelines for pulmonary embolism confirm most of the principles and recommendations of the previous guidelines from 2014; however, for diagnosis, they now recommend considering age-adapted thresholds for the D-dimer test and the YEARS criteria. For treatment of pulmonary embolism in hemodynamically stable patients, new oral anticoagulants (NOACs) are recommended as first-line therapy, except where specifically contraindicated, e.g., renal failure or lupus anticoagulant syndrome. In pregnant and breast feeding women, low-molecular-weight heparins (LMWHs) are still the only recommended treatment. For suspicion of pulmonary embolism during pregnancy, a newly developed management algorithm has been launched. According to the new recommendation for long-term follow-up after acute pulmonary embolism, in the presence of certain criteria, further diagnostic tests for chronic thromboembolic pulmonary hypertension (CTEPH) should be performed.
