Abstract:
In recent decades, medicine has undergone massive technical and scientific development. Unfortunately, particularly postgraduate medical training could not keep up with this development. Colleagues of all specialties complain about a lack of clinical training, especially in basic medical skills, medical history taking, and physical examination. The deficits are clearly visible in current training evaluations of the Austrian Medical Association. In addition, common factors which determine a good or a bad evaluation were defined. These include the implementation of a good training concept and the efforts of the person responsible for the training. Even in manual medicine, which has always been a discipline that relies on good physical examination, can this decline in skills be observed. A tangible generation conflict is also emerging. The older generations presuppose skills, the younger generations want to be trained in them. The passing on of experience-probably the most important medical tradition since the days of Hippocrates-falls by the wayside. In order to structure training and to pass on experience quickly, the use of checklists and simulation training has increasingly proven itself in medicine in recent years. The current article presents the "Ausserfern examination procedure", i.e., the systematic evaluation of an infant under consideration of neurologic, orthopedic, manual, and general medical aspects.
