Abstract:
Fetal segmental volvulus is a rare and potentially life-threatening condition. As prenatal segmental volvulus predominantly occurs without evidence of malposition findings are different from those in volvulus occurring during the neonatal period. Intrauterine segmental volvulus occurring during the third trimester may lead to prematurity, intrauterine growth retardation as well as unexpected severe fetal distress and probably miscarriage. This article reports a case of prenatal segmental volvulus presenting with hydrops fetalis and ascites due to fetal anemia resulting from bleeding into the necrotic loops of the small intestines. At about 22-26 weeks gestation ileal atresia with a V-shaped defect in the mesentery occurred in the fetus as an unrecognized primary intestinal incident. Torsion and strangulation of parts of the ileum through this V-shaped mesenteric defect were the secondary incidents. When confronted with hydrops fetalis, ascites and anemia physicians must take segmental volvulus into consideration.
