- Bismuth's and Corlette's classification of Klatskin tumours differentiates four different types of tumours according to their localisation, This classification influences the recommended surgical strategy for resection, However, in our opinion, the UICC tumour stage is of major importance if an indication for liver transplantation is being considered, Today, extended liver resections do not pose major technical problems. whereas impaired postoperative liver function may be life threatening, especially after extensive liver parenchyma loss or in patients with additional liver disease. The prognosis of postoperative liver function and the exclusion of extrahepatic tumour spread are crucial in the decision concerning the indication for surgical therapy. Preoperative chemoembolisation of the tumour and the liverparenchyma to be resected induces hypertrophy of the remaining liver parenchyma and is an option to make extensive liver resections safer, The development of new therapeutic strategies including multiple modality treatment in order to achieve better long-term results, and reducing the risk of impaired liver function are fields of major importance for the future.
