Abstract:
Nuclear medical imaging procedures, such as PET and SPECT are mainly used in nonlesional epilepsy in the presurgical diagnostic work-up. Even if MRI is inconspicuous, hypometabolism in FDG-PET is indicative for the epileptogenic focus, while in patients with limbic encephalitis, FDG-PET may also represent a mostly temporary hypermetabolism. Ictal SPECT shows a particularly high localization value in complex partial seizures lasting at least 15 s. Given that this diagnostic procedue requires several days of video EEG monitoring, it should be restricted to cases with non-lesional or discordant findings in other investigations. Ictal SPECT should be subtracted from interictal SPECT by postprocessing and co-registered with MRI (e.g. SISCOM) to increase the diagnostic yield.
