Abstract:
While remaining a demanding surgical procedure following a learning curve, right anterior minithoracotomy has been established as the most popular access for minimal invasive mitral valve surgery. In comparison to median sternotomy increased time of operation, cardiopulmonary bypass and aortic cross-clamping is accepted in exchange for a smaller skin incision. Reported perioperative rates of complication and mortality are similar in both methods as well as long-term outcome concerning residual mitral regurgitation and need for re-operation. Combining excellent operative quality with an improved cosmetic result, which can be essential for female and younger patients, right anterior minithoracotomy provides satisfaction on both, surgeons' and patients', sides.
