Abstract:
Atrial fibrillation (AF) and heart failure (HF) are common cardiovascular pathologies with severe prognostic implications that show bidirectional interactions. Current rhythm-control strategies using antiarrhythmic drugs in patients with HF do not improve outcome but may be indicated in case of significant AF-associated symptoms or tachycardiomyopathy. Amiodarone is currently the only antiarrhythmic drug generally available for patients with AF and HF. The 5-year success rate of 45% with amiodarone (prevention of AF recurrence) is limited by a 20% rate of extra-cardiovascular toxicity. Amiodarone is also a second line therapy option for rate control in AF with HF, and is also used in the acute setting of AF and HF, both for acute rate control as well as for acute rhythm control (cardioversion).
