What are the advantages of minimally invasive surgery for inguinal hernias in childhood?

Abstract:: For more than 15 years, laparoscopic inguinal hernia surgery in childhood has been controversial because the the results did not show any real benefits. In the meantime, however, technology has a developed safer and more efffective method. Considerable enhancements consist of the miniaturised instruments (3 mm) and the possibility of a "true" herniotomy. Benefits include unambiguous evaluation of both groins "at a glance", the simultaneous correction of a bilateral inguinal hernia without a further incision, and the low level of trauma to the spermatic cord because of the "internal" access. Because of the "true" herniotomy, i.e., the separation of the hernial sac as in the open procedure, the results have improved significantly. Current studies therefore recommend a minimally invasive procedure in children with verified bilateral hernia or if a contralateral (i.e., metachronous) hernia is suspected, and after incarcerated hernia. In these cases the child will benefit from the diagnostic and therapeutic possibilities of laparoscopy.
