Diagnostic Screening in Symptomatic Patients with and without Endometriosis

Abstract:
Purpose: Endometriosis is one of the most common benign diseases in young women and yet it remains underdiagnosed. Routine diagnosis is achieved by laparoscopic visualisation and biopsy of endometriosis lesions. However, meticulous history taking and psychometric tests might provide valuable information prior to the invasive diagnosis. Material and Methods: In a prospective explorative study, 158 consecutive symptomatic patients scheduled for laparoscopy were interviewed using a standardized protocol for patients' history and completed a psychometric questionnaire (Stressverarbeitungsfragebogen, SVF). The diagnosis of endometriosis was obtained by laparoscopy and histological examination of putative lesions and thereafter the patients were assigned to either the endometriosis or the control group. Results: At laparoscopy, 91 out of 158 (57.6%) patients were found to have endometriosis while 67 (42.4%) were disease-free. Endometriosis patients had significantly lower pregnancy rates (p < 0.05), significantly fewer births (p < 0.01) and their menstrual cycles were shorter compared to controls (p < 0.01). Significantly more patients suffering from endometriosis reported that dysmenorrhea was strongest on the first day of their menstrual cycle (p < 0.001). Endometriosis patients reported a significantly increased intake of NSAIDs compared to controls (p < 0.05). Dyschezia was observed more often in the endometriosis group (p = 0.07) especially during menstruation (p = 0.06). The tendency to take medication or stimulants under stress or in conflict situations as parameter of the SVF was significantly higher in the endometriosis group compared to controls (p < 0.05). Conclusion: These findings suggest that screening for selected patients' characteristics through meticulous history taking and psychometric tests provides valuable information prior to laparoscopy and is useful when offering individually optimized counseling and therapy to each patient.
