- Rare variants and simulators of angioedema Although clinically similiar with abrupt and short-lived non-pitting swelling of the skin and/or mucous membranes, angioedema (and simulators) can be caused by a variety of elicitors, depending on different pathophysiological pathways. In the last decade, research and review papers focused on the fundamental role of bradykinin in hereditary as well as angiotensin-converting enzyme inhibitor-induced angioedema. In this paper rare entities with angioedematous symptoms and simulators of angioedema are reviewed: the cytokine-associated angioedema syndromes including episodic (cyclic) angioedema with eosinophilia (Gleich's syndrome) and non-episodic angioedema with eosinophilia, NERDS (nodules, eosinophilia, rheumatism, dermatitis and swelling), Clarkson syndrome (capillary leak syndrome) as well as simulators like vena cava superior syndrome and several inflammatory diseases. Although these variants and simulators of angioedema share some common (clinical) features, they have important differences in pathophysiology, that consequently lead to fundamentally different diagnostic and therapeutic measures.
