- Rare contact allergens - rare or merely seldom identified? Allergic contact eczema is diagnosed on the basis of a patch test. Test substances should be selected according to the patient's medical history. More than 4,000 chemicals have been reported to trigger contact allergies. Appropriate selection of test substances requires knowledge, experience, and the shrewdness of a sleuth. What makes the task difficult is the fact that the large majority of allergens have been described as triggers in just isolated cases, are not commercially available for test purposes, are not declared in the finished product, or are not known to be cause of sensitization at all. These are the reasons why testing physicians are not aware of many allergens, which are hence not detected as triggers of eczema in the test setting. Furthermore, a fact scarcely known in clinical routine is that even substances with a weak sensitizing potency may cause sensitization in conditions of high exposure, whereas substances with a high allergic potential are usually kept under lock and key" and therefore subject to a very low likelihood of contact. Some aspects of rare allergens or rare sensitization will be presented here as examples. As regards the diversity and large number of allergens in various products of daily living, we refer to the specific published literature on these.
