Quality assessment of cataract surgery of the Department of Ophthalmology, Medical University of Graz

- BACKGROUND: There is a high quality demand on cataract surgeons. Despite an almost complete standardization of the procedure, some complications may occur. Most important is the rupture of the posterior capsule because of possible postoperative deterioration of the visual acuity. We investigated the rupture rates without and with vitrectomy of the past 13 years of the Department of Ophthalmology, Medical University of Graz. MATERIAL AND METHODS: In the operation data base we searched for "rupture without vitreous loss", "rupture with vitreous loss", "surgeon" and the for the contents of the free-to-enter text field "Remarks". RESULTS: Within the past 13 years 36438 phacoemulsifications and 3357 extracapsular extraction were performed. Only files from those surgeons were extracted who did at least 200 operations (216-9 057). After correction of primary intended opening of the posterior capsule 36329 phacoemulsifications remained, in which 712 (1.96 %) ruptures had occurred. The individual rupture rate of the surgeons was between 0.41-7.63 %. CONCLUSIONS: The rate of ruptures of the posterior capsule is between 4.6-10 % in the recent literature. We were within the upper limit. In our assessments we found no correlation to the amount of operations having been done or to the time of experience of cataract surgery. The reasons for the ruptures were identical to those cited in the literature. Additional factors like quality of education, personal stress or misjudgement may play a role. A preoperative risk assessment, a step-by-step education and a continuous check-up of the results may decrease the amount of ruptures.
