Basaloid carcinoma of the high rectum - A case report and review of the literature

- A case of basaloid or so-called cloacogenic carcinoma of the high rectum associated with a tubular adenoma showing low grade dysplasia is reported. Colonoscopy had revealed an exophytic tumour in the high rectum of a 80-year-old woman with digestion discomfort. Multiple liver metastases were observed on computed tomography. Histologically, the tumour was formed of nests of small basophilic cells showing central areas of necrosis and small foci of keratinisation, consistent with the diagnosis of a basaloid or so-called cloacogenic carcinoma, as were the ultrastructural aspect and the immunohistochemical profile. Basaloid carcinomas are exceedingly rare in this localisation, and the association with an adenoma has not been described so far. They probably origin from pluripotent glandular stem cells. Although associated with a tubular adenoma, there was no convincing histological evidence that the carcinoma may have arisen within it. Differential diagnosis of basaloid carcinoma at this site including poorly differentiated neuroendocrine carcinoma as well as secondary, metastatic tumour is discussed.
